NAVIGATOR Substance Use Disorder Treatment CRI-Help is a substance
use disorder treatment center with several locations across Los
Angeles County. We've been around since 1971 and are noted as an
organization practicing integrity in our commitment to ethical and

the community by providing first-class substance use disorder
treatment to adults and families seeking freedom from the bondage
of addiction, and we live out that mission everyday with our staff

openings at all sites. The basic function of the Housing Navigator
is to provide housing-focused supportive services for Persons
Experiencing Homeless (PEH) with the goal of helping them identify,
apply for, secure, and move into permanent housing. The Housing
Navigator will work with participants while they are enrolled in
RBH or RH to ensure they have a safe and stable housing plan ready
for when they are discharged from these settings. Responsibilities
Under the supervision of the Housing Navigation Supervisor, conduct
comprehensive clinical assessments, and provide intensive case
management services. Under the supervision of the Housing
Navigation Supervisor, develop individualized IHSSP (Individualized
Housing and Supportive Services) plans with measurable goals and
objectives, and assist participants in achieving their desired
outcomes. Coordinate, prepare and maintain required charting and
documentation in a timely and thorough manner, within program
guidelines. Develop relationships with representatives in other
agencies to support individuals in attaining services such as
housing, mental health care, medical treatment, financial
assistance, legal advocacy, and other community resources. Assist
with preparation housing applications and other required
documentation. Network and collaborate with housing providers,
landlords and property managers to develop housing stock for
program, which includes temporary and permanent housing
alternatives. Perform home visits within scheduled time frame,
assess for needs on a continuous basis, and refer clients to
services within their community. Provide support, and link clients
to, Electronic Homeless Programs, including CES and HMIS. Maintain
an active account with the HMIS. Under the supervision of the
Housing Navigation Supervisor, provide clinical intervention
services, crisis intervention, and conflict resolution, and
continually monitor at-risk / high-risk situations that require
clinical interventions. Conduct advocacy services and ensure
clients receive all entitled benefits. Participate in daily debrief
sessions, weekly supervision, clinical supervision, department and
agency-wide meetings, and training and development opportunities,
as determined appropriate by the supervisor. Submit weekly /
monthly reports, such as mileage, weekly schedules, quarterly
training reports, corrected audit findings, and status of unhoused
individuals. Implement and consistently practice harm reduction,
trauma informed care, housing first and integrated care
interventions. Work in a multi-disciplinary team environment,
utilizing professional, open and respectful communication. Maintain
a solution focused, strengths-based and client-centered approach
with managers, co-workers and clients. Ensure performance
management standards are met through ongoing supervision,
addressing barriers as they arise. Maintain client records
according to HIPAA, and adhere to all client confidentiality
requirements and standards. Comply, and maintain familiarity, with
agency and program policies and procedures and other applicable
regulations. Other duties as assigned by the Housing Navigation
Supervisor. Skills Familiar with the principles of Harm Reduction,
Motivational Interviewing, Trauma Informed Care, Housing First and
Integrated Care. Ability to effectively present information and
respond to questions from clients, office personnel, other
employees and the general public. Ability to work in fast-paced,
high energy environment. Ability to define problems and use
critical thinking skills to identify solutions. Ability to work
independently and with others including other employees, clients
and members of the public in face-to-face and telephonic contexts.
Ability to multi-task, establish priorities and manage time, meet
deadlines and perform duties under time constraints. Ability to be
flexible and adapt to changing work demands. Maintain high level of
concentration and attention to detail for extended periods of time.
Ability to respond effectively to sensitive inquiries or concerns.
Maintain a high level of ethical and professional standards in
accordance with agency policies. Computer literate (basic working
knowledge of Microsoft WORD, Excel, and Outlook) and data /
electronic records. Education / Training High School Diploma / GED
2 or more years of experience working in housing, and with
culturally diverse populations, including co-occurring and
homeless. 1 or more years of experience providing Housing
Navigation or Case Management License / Certification Requirements
Preferred: Individual be registered, or certified, with one of the
state-approved addiction counselor certification entities (CADTP,
CAADE, or CCAPP). Special Conditions Must have a valid California
driver's license, properly registered vehicle and insurance. Must
be willing to undergo random drug screenings. Working Conditions
Occasional ascending and descending stairs. Frequent sitting for
prolonged periods. Occasional crouching below the waist and / or
above the shoulders. Frequent lifting, and / or pushing / pulling
up to 10 pounds. Occasional lifting, and / or pushing / pulling up
to 25 pounds. Frequent operation of motor vehicles. Frequent work
may be performed at various locations across Los Angeles County.
