OF POSITION for CASE MANAGEMENT Provides case management services
for an assigned caseload. Works closely with individuals we serve
and/or their families. Communicates with staff, vendors and the
community. Organizes and resolves work-related situations.
ESSENTIAL DUTIES AND RESPONSIBILITIES for CASE MANAGEMENT include
the following. Other duties may be assigned. Case Manager contacts
assigned individuals we serve and documents; contacts according to
established SCLARC Standard Operating Procedures (SOPs) and policy,
and completes required documentation. Case Manager prepares case
transfers as needed. Case Manger attends all unit, division,
supervision, staff and in-service training meetings as required.
Case Manager attends all Planning Team meetings at primary programs
and any other individual we serve related special meetings/hearings
(e.g. SCAN, court, IEPs/IFSPs staffings, mental health,
governmental/community agencies, placement, etc.) as required. Case
Manager handles officer-of-the-day (OD) duties as required. Case
Manager handles confidential information and adheres to Welfare and
Institution (W&I) codes. Case Manager consults with appropriate
specialists regarding the service needs of individuals we serve.
Case Manager maintains effective working relationships with top
management, peers and staff. Case Manager may act as Program
Manager in his/her absence. SUPERVISORY RESPONSIBILITIES This
position has no supervisory responsibilities. However, one may be
asked to act as Manager in the absence of the Program Manager.
QUALIFICATIONS To perform this job successfully, an individual must

essential functions. EDUCATION and/or EXPERIENCE Bachelor's degree
(B. A.) from four-year college or university; two years related
experience and/or training; or equivalent combination of education
and experience. Internships and/or training level employee may have
less requirements. OTHER SKILLS AND ABILITIES Demonstrates cultural
sensitivity and awareness to diverse groups. Must be able to handle
confidential information and adhere to Welfare and Institution
codes. Must have good time management skills and be PC literate.
CERTIFICATES, LICENSES, REGISTRATIONS California Driver License,
Class C with personal auto insurance (meeting minimum California
requirements).
