Job Description:
A company is looking for a Discharge Care Management Nurse RN
Remote in Central or Eastern Time Zone.
Key Responsibilities
Evaluate member discharge needs and develop readmission prevention
plans
Coordinate care post-discharge with providers and members
Document discharge activities and participate in team meetings and
education discussions
Required Qualifications
Associates or Bachelor of Science in Nursing
Current, unrestricted Compact RN license in the State of
residence
5+ years of recent experience in the inpatient acute setting
2+ years of experience in discharge planning
Experience with multiple health insurance products within the
insurance industry
