Job Description:
A company is looking for a Clinical Pharmacist Market Support
for the Clinical Excellence Team, working remotely.
Key Responsibilities:
Conduct telephonic patient outreach and provider-facing activities
to improve STARs measures
Analyze STARs and HEDIS reporting to provide insights and best
practices to provider groups
Perform annual medication reviews and develop patient-centered
medication consultation plans
Required Qualifications:
Doctor of Pharmacy degree or equivalent experience
Active, unrestricted pharmacy license in state of primary residence
and TN
5+ years of clinical pharmacy practice experience
2+ years of experience with Medicare STARs & HEDIS measures
2+ years of provider-facing interaction experience
