Job Description:

A company is looking for a Health Coach - Wellness/Diabetes.
Key Responsibilities

Assist members by completing health assessments and implementing care plans for chronic conditions
Provide telephonic support and coaching for wellness care plans to improve members' health
Monitor wellness plans and recommend changes based on member needs and outcomes

Required Qualifications, Training, and Education

Associate's degree and 2 - 4 years of related experience
Registered Dietitian, Physical Therapist, Respiratory Therapist, RN, Master's in Behavioral Health or Health Coach Certification required
Certified Diabetes Care and Education Specialist (CDCES) certification required upon hire for Enterprise Population Health
Oklahoma residency required
