Tierrasanta, La Mesa, Santee, Lakeside, El Cajon and surrounding

more! Qualifications:
- Must be a Registered Nurse with a valid state license
- One year of experience working with your RN license
required
- Hospice experience preferred, but not required
- This role requires a valid driver's license and a good driving
record as a condition of employment. Associates must comply with
all state and federal driving regulations
- Associates in roles requiring driving are responsible for
maintaining a valid and active driver's license. Failure to
maintain a valid license may result in reassignment, suspension, or
termination in accordance with company policies and applicable
laws
- Reliable transportation and willingness to drive to patient's
location
- Must be able to lift 50 pounds
- Must clear a criminal background check, physical and drug
screening
- You may be required to work outside of your regular hours
depending on business needsWhat you'll be doing:
- Develop a detailed Plan of Care for assigned patients ensuring
all services needed are identified and met while working closely
with the Interdisciplinary Group, patient, family and attending
physician
- Responsible for the coordination of care for the patient and
family, providing skilled Hospice and Palliative care for the
remainder of patient's life
- Provide comprehensive nursing evaluations and assessments
- Provide field supervision to LVN's and Home Health Aides
assigned to your patients
- Filed case management
- Other duties as assigned Anticipated pay range: $55 USD - $58
